2023 - 24 Enrollment Registration

Hello! We are looking forward to an amazing 2023-2024 school year at Adventure Kids Academy and hope that you are as excited as we are to start (or continue) your child’s journey into the world of education.
 

Please fill out the required information, carefully read the enrollment Terms and Conditions below and digitally sign the agreement.

TERMS AND CONDITIONS

  • Registration fees for the school year are $100 per child and non-refundable once your child has been accepted into the program. The registration fee must be paid when submitting this form before your child’s enrollment will be accepted. Once accepted, you will be contacted via email to confirm your child’s enrollment.
     

  • If we are unable to accept your child’s enrollment due to limited space, your registration fee will be refunded and you will be automatically placed on our waiting list.
     

  • September (or first month if enrolling after the school year has begun) and June tuition fees are due by August 15, 2023 or upon notice of enrollment for mid-year enrollees. These first and last month fees are non-refundable.
     

  • Tuition is charged as a flat rate fee on a monthly basis and has been averaged over the entire school year (September –June), factoring in holidays and breaks. Tuition remains the same from month to month regardless of absences, family vacations, weather, or other circumstances outside of our control.
     

  • If 2 or more siblings are simultaneously enrolled, a 5% discount on monthly tuition fees will be given to the youngest child.
     

  • Tuition is due on the 1st of each month (or the first business day of the month if the 1st falls on a weekend). Once tuition is paid, it is non-refundable. Early withdrawal during the school year requires one month (30 days) advanced written notice.
     

  • A $25 late fee will be charged for all monthly tuition payments made after the 1st of the month (or the first business day of the month if the 1st falls on a weekend). A 5-day grace period will be given, at which point your child will not be allowed to attend class until outstanding fees have been paid.
     

  • If outstanding fees have not been paid after a period of one month, at our discretion, we reserve the right to refer your account for collection.  In this event, you will be responsible for all costs of collection, including but not limited to our attorney fees as well as all late fees.
     

  • A $35 fee will be charged for all NSF checks.
     

  • Tuition payment checks may be mailed or placed in the lobby lock box (located outside each classroom). Cash tuition payments need to be in a sealed envelope with your child’s name and must be placed in the lobby lock box to guarantee credit to your account. Adventure Kids Academy bears no responsibility for tuition payments that are made in any other manner.
     

  • Parents are expected to pick up their child when class ends.  A 5-minute grace period will be given, at which point you will be charged $5 for every 5 minutes late.  This must be paid directly to the teacher, NOT the school.
     

  • Valid emergency contact information is required as part of a child’s enrollment. We must be able to reach you or a pre-arranged emergency contact at all times. You authorize us to provide first aid and/or seek immediate medical care in case of an emergency. Such care may be from a physician or hospital other than your child’s physician. You also authorize us to determine what constitutes an emergency.
     

  • Each child must be up to date with immunizations and will be required to provide documentation from their primary care physician on the first day of school.​

  • Children who are ill are not allowed to attend school (to be determined at our discretion and/or in accordance with our policy regarding ill children, as stated in the Parent Handbook). We reserve the right to request a doctor’s note before children can return to school. We will notify you if your child becomes ill - a parent, guardian or pre-arranged emergency contact must be able to pick them up within 30 minutes of the notification.

  • You agree to be liable for any loss, damage or destruction of any property of the school caused by your child, and for any damages for which the school becomes liable or charged because of you or your child’s actions, or the actions of family or acquaintances who may accompany your child to, from, or during school, or any of its destinations or events.

  • Adventure Kids Academy reserves the right to suspend or dismiss a child/family at our sole discretion, for reasons including but not limited to; unsatisfactory or inappropriate behavior, inadequate expertise or resources for the child’s educational, medical, or personal needs (subject to any applicable regulatory requirements), violations of our policies, or any other reason that conflicts with the best interest of the school or its student body. Suspension or dismissal may be with or without advanced notice, and any refunds or fee forgiveness with be solely at our discretion at the time of such action

  • You authorize us to use you and your child’s name, email, home address, and phone numbers as needed for school-related communications. All such information will be used solely for school-related purposes and will not be shared otherwise unless you provide us with express permission to do so

  • You authorize us to use the likeness, photographic imagery and/or video footage of your child for Adventure Kids Academy marketing purposes.  This includes but is not limited to print, website and social media.  Your child’s name will be withheld from these uses.
